• Facilitators are required to complete the Post-Program Survey.   This survey captures data relating to the implementation of each class, including any challenges facilitators may have encountered.

Process

• Facilitators must complete a separate Post-Program Survey for every club they lead. Facilitators should complete this survey immediately after the program ends.

• This survey is completed and submitted online.
